Title IX Incident Reporting Form

 Official Notice

Anyone can report an incident(s) of sex discrimination, sexual harassment (including sexual violence), or sexual misconduct. Importantly, an alleged victim may file anonymously. However, an anonymous report or complaint may impair the ability of the University to provide resources to affected parties—such as Supportive Measures—or to investigate allegations of Sexual Misconduct. All investigations will use the preponderance of the evidence (more likely than not) standard to determine violations of the Sexual Misconduct Policy.
Complaints or allegations of sex discrimination, sexual harassment (including sexual violence), or sexual misconduct will be handled by the Title IX program.

Responsible Employee

A Responsible Employee may utilize this form to report an incident. For more information on who is a Responsible Employee and the requirements for reporting, please see Texas Education Code § 51.251 and § 51.252.

Supportive Measures

Any person (when applicable) who is reported as an alleged victim of sexual misconduct will be promptly informed by the Title IX Coordinator and offered Supportive Measures whether or not a Formal Complaint has been filed. Importantly, the University will take into consideration the alleged victim’s wishes when offering Supportive Measures. Supportive Measures are reasonably available individualized services, without any fee or charge, “designed to restore or preserve equal access” to an alleged victim’s education or employment. Also, Supportive Measures are non-disciplinary and non-punitive measures that do not unreasonably burden the other party.
Supportive Measures may include but are not limited to resources and important information regarding counseling services, extensions of deadlines or other course-related adjustments, modifications of work or class schedules, withdrawal from or retake of a class without penalty and upon availability, campus escort assistance, mutual restrictions on contact between the parties, leaves of absences, increased security, and monitoring of certain areas of campus, or other similar measures tailored to the individualized needs of the parties.
Importantly, the University will maintain the confidentiality of Supportive Measures provided to the parties to the extent that maintaining such confidentiality does not impair the ability of the University to provide the Supportive Measures.
